At the bottom of the overall list in the Expat City Ranking, Milan, Italy, secured the 49th position, making it the lowest-ranked city. Rome, Italy, followed closely at number 48, with Vancouver, Canada, at the 47th position.

Expats in the two Italian cities, Milan and Rome, faced common challenges, resulting in bottom 10 rankings in four out of five survey indices. The key areas of dissatisfaction included the low quality of life and poor work conditions. Vancouver, on the other hand, emerged as the city where expats reported the least happiness (49th) and encountered the most difficulties with their personal finances (49th).
